What Causes Brain Fog? The Neuroscience of Why You Cannot Think Clearly

Brain fog has a biology
Brain fog is not lazy thinking or normal ageing. It is a specific neurobiological state in which the prefrontal cortex - your executive function center - gets metabolically deprioritised.
The mechanism is straightforward: when the autonomic system is dysregulated, the brainstem reroutes energy and oxygen away from the cortex and toward survival circuits (limbic system, brainstem, sympathetic outputs). Thinking clearly is no longer the priority. The system wants you to be alert to threat, not to write nuanced emails.
The downstream cognitive symptoms - forgetting words mid-sentence, re-reading the same paragraph, decision fatigue, can't follow a conversation - are the cost.
The 7 most common causes, in order
1. Nervous system dysregulation (most common, least diagnosed).
Chronic sympathetic activation or dorsal vagal shutdown both produce brain fog. The prefrontal cortex needs ventral vagal baseline to function fully.
2. Sleep architecture failure.
Slow-wave (deep) sleep is the period during which the glymphatic system clears metabolic waste from the brain, including amyloid and tau. Lose slow-wave sleep for a few nights and brain fog is guaranteed. Lose it chronically (which is common in fibromyalgia, ADHD, perimenopause, sleep apnoea) and brain fog becomes the baseline.
3. Neuroinflammation.
Microglial activation - the brain's resident immune cells - produces brain fog directly. Long COVID, autoimmune flares, post-viral states, and chronic stress all trigger microglial activation.
4. Blood sugar volatility.
Glucose is the brain's primary fuel. Spikes and crashes - especially the crashes - produce brain fog in real time. Refined carbs without protein/fat are the largest single dietary driver.
5. Dysautonomia / POTS.
When standing causes cerebral hypoperfusion (less blood to the brain), brain fog is one of the first symptoms. Often confused with anxiety.
6. Hormonal disruption.
Perimenopause, postpartum, thyroid dysfunction, low cortisol curve, low DHEA, low testosterone - any of these produce brain fog. Cycle-tracked brain fog usually has a hormonal driver.
7. Post-viral cognitive impairment.
Long COVID, ME-CFS post-onset, post-mono, post-Lyme. The biology overlaps with neuroinflammation but the trigger is identifiable.
How to find out which one is yours

- Protect slow-wave sleep. Dark, cool room. No screens 60 minutes before bed. Consistent wake time.
